29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   The employer does not ensure employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were protected by guardrail systems, safety net system, or personal fall arrest system. This violation was observed on or about August 14, 2014, on the roof of a house where employees were exposed to a fall of more than 6 feet when performing roofing activities without a fall protection system.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees engaged in residential construction activities 6 feet or more are protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   The employer does not ensure that ladders used by employees are extended at least 3 feet above the upper landing surface. This violation was observed on or about August 14, 2014 at the construction site on the east side of the house where employees were exposed to fall hazards while using an extension ladder to access a roof without extending the side rails at least 3 feet above the roof landing.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders used by employees have the side rails extended at least 3 feet above the roof landing.
