29 CFR 1926.451(b)(4): Each end of a platform, which was not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches: (a) On or about August 13, 2014, for employees exposed to fall hazards of approximately 25 feet where one employee suffered a fatal fall while laying brick from a cantilevered platform of a fabricated frame scaffold at 201 South Phillips Avenue in Brookings, South Dakota.
