29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee attached. a) Jobsite: Sixty foot catwalk over aeration tank#1 An employee used a 3 foot steel beam that was secured with a C clamp on one end as the anchorage point. The beam and the C clamp became loose and the employee fell 19 feet, on or about 8/14/14.
