29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Three employees were working on a "Steep roof" work at a pitch of 5/12 with elevations between 22 feet and 30 feet without the use of proper fall protection or any fall arrest system.

29 CFR 1926.602(c)(1)(vii): Unauthorized personnel shall not be permitted to ride on powered industrial trucks. A safe place to ride shall be provided where riding of trucks is authorized. The employer had watched his employee mount the material handling platform attached to the rough terrain forklift, the employee rode the platform from the rooftop as he was lowered by the employer, the employer viewed the employee while he rode the platform not designed for personnel.
