29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Employee(s) in the Prep / Insert Room are provided & required to use elastomeric air-purifying respirator(s) by the employer as personal protection from organic vapors during adhesive spraying of parts as well as Methylene Chloride vapors during immersion & retrieval of parts to/from the degreasing tank. A written respiratory protection program was not implemented which provided for medical evaluations of user(s) as well as guidance for respirator care & maintenance; exposing Insert / Prep worker(s) to health hazard(s) from aggravation of existing cardiac and/or pulmonary disorder(s) as well as skin conditions like dermatitis. Condition(s) were noted on or about 8/14/2014.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a) Employee(s) in the Prep / Insert Room are provided & required to use elastomeric air-purifying respirator(s) by the employer as personal protection from organic vapors during adhesive spraying of parts as well as Methylene Chloride vapors during immersion & retrieval of parts to/from the degreasing tank. Each respirator user was not evaluated as to their medical suitability to wear a respirator; exposing them to health effects of aggravation of cardiac and/or pulmonary disorders. Note: The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.
