29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) On or about August 12, 2014 at the jobsite located at 768 W. Jackson Street, Chicago, IL. - employees performing masonry activities on the fourth floor of a rehab were exposed to falls of approximately ten (10) to twelve (12) feet created by the presence of unprotected floor holes.
