Employee did not utilize LOTO procedures developed for the machine.

29 CFR 1910.147(f)(1): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed: a) On or about 08/15/2014 a maintenance employee was troubleshooting a defect on the hot dog line packaging machine and received a disabling injury that resulted in the amputation of employee's hand. The employee utilized no energy control procedures during the troubleshooting process.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Additional guarding was needed to protect employees from reaching into S cage fingers on loading head for hot dog line. b) Additional guarding was needed to protect ingoing nip points which were only top guarded for in-feed conveyor to loading head. c) Current guard for loading head was not secured to equipment with special tools or interlocked to prevent operation when it was not in place. d) Guard removed by mechanic before incident was only secured with finger tight bolts which did not require a tool for removal.
