29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide:   Southside of residence located at 4971 Ansbury Place, Acworth, Georgia. The unprotected plank walkway, which spanned between the fabricated frame scaffold system and the roof, was not at least 18-inches in width, exposing an employee to fall hazards of between approximately 12-feet 11-inches and 13-feet 6-inches.

29 CFR 1926.451(e)(6): Integral prefabricated scaffold access frames were not specifically designed and constructed for use as ladder rungs;   a) Southside of residence located at 4971 Ansbury Place, Acworth, Georgia. An employee was exposed to a fall hazard of 12-feet 11-inches as there was not a means of accessing the working platform of a fabricated frame scaffold system safely. The employee was climbing the end frames of the scaffold system as if they had been designed and constructed for use as ladder rungs.

The upper most working platform of the fabricated frame scaffold system was not protected by guard rails or safety nets. An employee, who was not wearing a personal fall arrest system, was exposed to a fall hazard of approximately 12-feet 11-inches.

The plank walkway, which spanned between the fabricated frame scaffold system and the roof, was not protected by guard rails or safety nets and the employee was not wearing a personal fall arrest system, exposing an employee to fall hazards of between approximately 12-feet 11-inches and 13-feet 6-inches.

An employee, who was not wearing a personal fall arrest system, nip pointing mortar joints from an unprotected steep sloped (6 in12 pitch) roof was exposed to fall hazards of approximately 10-feet 8-inches.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:   a) Southside of residence located at 4971 Ansbury Place, Acworth, Georgia. An employee was exposed to a fall hazard of approximately 12-11-inches as the scaffold system did not have a firm foundation. None of the legs of a fabricated frame scaffold system were bearing on base plates. The legs of the scaffold system were bearing directly on wooden mudsills.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which secure vertical members together laterally.   a) Southside of residence located at 4971 Ansbury place, Acworth, Georgia. The outer panel of the upper section of the fabricated scaffold system was not braced. Without proper bracing the vertical members of the scaffold were not secured together. Cross bracing also aligns the members ensuring the scaffold is plumb, square, and level. The employee working from the scaffold system was exposed to a fall hazard of approximately 12-feet 11-inches.

An employee working from an unprotected steep sloped (6 in12 pitch); roof had not received fall protection training from his employer prior to being exposed to a fall hazard of approximately 10-feet 8-inches.
