29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: a.) Bison Laboratories' production area - On or about 8/18/14, temporary employees dispense and fill containers of sodium hypochlorite, a corrosive. Remedy did not verify that the workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment, for required use of PPE for workplace tasks. The host employer, Bison, provides a variety of personal protective equipment (PPE) for the employees to use, including: half face gloves; face shield; and safety glasses. ABATEMENT CERTIFICATION REQUIRED

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: a) Bison Laboratories, Inc. - On or about 8/19/14, Remedy employees fill containers of sodium hypochlorite, a corrosive liquid. The employees do not use goggles. Employees use safety glasses or prescription glasses with disposable plastic side shields which did not form a protective seal around the eyes and prevent corrosive liquids from entering around the glasses. The employer did not ensure that each affected employee used appropriate eye and face protection when exposed to splash hazards from liquid chemicals. ABATEMENT CERTIFICATION REQUIRED
