29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. The Weave Stop, LLC - on or about August 19, 2014, the entrance/exit area was not maintained in an orderly condition. A loose cable was on the floor approximately 40 inches from the doorway, exposing employees to a trip hazard while entering and exiting the building. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: The Weave Stop, LLC - on or about August 19, 2014, a fire extinguisher was not mounted on the wall, but was placed on the floor and was not readily accessible behind a chair and work station. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
