29 CFR 1910.134(e)(1): General. The employer shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace. The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.   a)Paint booth and area:  On or about August 19, 2014, and prior to, employees were exposed to inhalation hazards from chemicals in paints to include toluene, xylene and Di Methyl Ether while conducting spray painting work tasks with required use of full face and half mask respirator and not provided a medical evaluation for the respirator.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:   a)Maintenance and other area:  On or about August 19, 2014, and prior to, employees were exposed to inhalation hazards from chemicals and dust while conducting work tasks and voluntarily wear dust masks without the training of appendix D or equivalent.
