29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.    a) Production Work Area, Spiral Rolling/Winding Machine, Work Platform:  On or about August 7, 2014, an employee performed various job tasks on a work platform that was no less than 6 feet above ground surface and with an unguarded open-side, exposing employees to a fall hazard.

29 CFR 1910.132(d)(1): The employer did not assess a new piece of machinery to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):  a) Production Work Area, Spiral Rolling/Winding Machine:  On or about August 7, 2014,  employees operated and or performed various job tasks around a piece of equipment/machinery (Spiral Rolling/Winding Machine) designed by the company without the employer doing an assessment to determine hazards present or likely to be present that would necessitate the use of personal protective equipment.
