29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:    a) 65 Sturgis Road, Edison, NJ 08817. The employer allowed his employee operate a Hitachi pneumatic coil nailer to install new roofing shingles when the employee was not wearing any eye and or face protection.     Condition occurred on or about 08/15/14.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) 65 Sturgis Road, Edison, NJ 08817. The employer allowed his employees to work on and from the roof of a two story, single family home with no fall protection systems in place.     Condition occurred on or about 08/16/14.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:  a) 65 Sturgis Road, Edison, NJ 08817. The employer allowed his employees to access the roof of a two story, single family home when the portable aluminum ladder was not being used as intended. At the rear of the home, employees used the fly section of a 28 foot aluminum ladder, positioned upside down, to access the roof.    Condition occurred on or about 08/16/14.
