29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:   On or about 8/20/2014, in the paint shop on the south side of the facility, the employer allowed employees to apply bed liner containing methylene bisphenyl isocyanate (4,4' Diphenylmethane diisocyanate) without ensuring that employees wore protective clothing to prevent skin contact.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  On or about 8/20/2014, in the paint shop building on the south side of the facility, the employer required employees to wear full-face and/or air supplied respirators while painting and applying bed liner without establishing an adequate written respiratory protection program.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  On or about 8/20/2014, at the facility located at 1922 Cheryl Drive in Abbeville, LA, the employer failed to provide training regarding the new label elements and safety data sheet format to employees who work with hazardous chemicals such as isocyanates.
