29 CFR 1926.50(c): In the absence of an infirmary, clinic, hospital, or physician, that was reasonably accessible in terms of time and distance to the worksite, which was available for the treatment of injured employees, a person who has a valid certificate in first-aid training from the U.S. Bureau of Mines, the American Red Cross, or equivalent training that can be verified by documentary evidence, was not be available at the worksite to render first aid: At the project no one had a valid certificate in first aid training and the nearest medical facility was more than four minutes away. Employees used circular saws, skidsteers, and hammers to pound nails without eye protection onsite. They were exposed to the hazards of being struck by the saws, skidsteers, and nails. Employees exposed to these hazards can suffer serious injuries such as, but not limited to, deep cuts, compound fractures, and eye impalement. Without personnel properly trained to render appropriate first aid until emergency medical services begin, these serious injuries can lead to more severe consequences such as death, loss of limb, and blindness.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: Near the south side of the gym under construction eye protection was not used while using circular Skilsaws to cut lumber and hammers to pound nails into lumber to build forms for concrete. Employees were exposed to the hazard of being struck in the eyes by wood splinters and nails.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage: In the driveway and parking area on the eastside of the project a 110 VAC extension cord was not protected from damage as it lay on the compacted soil and was run over by vehicles. There were tire tracks in the soil and over the cords. The cords powered Milwaukee drills and circular Skilsaws used to build forms for concrete. Employees were exposed to the hazards of contacting electricity.
