29 CFR 1926.451(b)(3): The front edge of platforms were more than 14 inches (36 cm) from the face of the work where guardrail systems were not erected along the front edge and/or personal fall arrest systems were not used in accordance with paragraph (g) of this section to protect employees from falling:  Jobsite:  Employees exposed to gaps of 16" to 18" inches on the front edge of the working platform.

29 CFR 1926.451(h)(1):  Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contained or deflected the falling objects:  Jobsite:  Employees storing material on a working deck 25 feet off the ground with no toe-boards.
