OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards.     Location: 2nd Ave Tunnel Project, 96th Street and 2nd Ave, New York, NY 10019    On or about August 20, 2014    a). Employees were cleaning the 6" concrete pump slick pipes after a planned slab pour.  Employees did not follow the recommended procedures for cleanout of the concrete pump pipes.    1). A catch basket was not installed at the end of the flexible line.  2). The flexible rubber hose was not disconnected from the slick pipe during cleaning operations.   3).The placing line was not securely fastened to prevent movement.  4). Air Pressure gauge was broken/inoperative.    Among other methods, feasible means to abate the hazard include; a). Following proper procedures as outlined in Safety Bulletin from the American Concrete Pumping Association Dated January 2010, pages 1 through 6. b) the Flexible hose should be detached from the steel slick lines prior to pressurizing the lines with air. c). A basket is to be affixed to the last slick line to catch cement and the foam ball cleaner (rabbit).  d). The slick line is to be secured in place to prevent any movement. e). Ensure all devices/meters/gauges are in proper working order.
