29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.     Worksite: The employee working from the carpenter bracket scaffold was not properly trained to recognize the hazards of working from the scaffold. The employee stepped on the top guardrail and section of the roof to pull a sheet of plywood. Consequently on or about 8/21/2014 the employee stepped above the rails of the scaffold and fell to the ground sustaining serious injuries.
