29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process: a) Wet process production D wing - On or about august 12, 2014, the employer operated a PSM (Process Safety Management) covered pigment dispersion process with a capacity of greater than 10,000lbs of flammable liquid, as defined in 1910.119(a)(1)(ii). The piping and instrument diagrams for the process that included PMD-D2, CT1, and CT2, had not been updated to reflect changes made to the system since the installation represented on the OEM drawings.
