OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. On or about March 11, 2014, and at time prior thereto, the employer did not ensured that the most current records of capacity verification of each anchor and date of verification be on a weather resistant tag securely attached to every anchor that has been tested. This condition exposed employees to struck-by hazards from the derrick of the rig.
