In energized installations each outlet box did not have a cover, faceplate, or fixture canopy: a) In the gymnasium area; The outlets and the light switches in the gymnasium area were not protected with adequate faceplates. i. The outlet and light switch on the gymnasium wall and ii. The light switch with the four switches on the gymnasium wall.
