29 CFR 1910.133(a)(1): Protective eye and/or face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    a)  Laboratory 1005:  An employee heating a potassium cyanide solution was exposed to potassium cyanide that reacted by violently erupting out of a beaker and hitting the employee in the face and mouth.   The employee was not required to wear appropriate eye and face protection (e.g., chemical splash goggles and face shield) when performing this procedure; on or about 7/8/14.     b) Laboratory 1005 - Walk-In Hood:  Employees dispensed acetonitrile and methanol and were not required to wear appropriate eye protection (e.g.,  chemical splash goggles or equivalent) ; on or about 7/10/14.    c) Laboratory 1005:  Employees who poured and dispensed various hazardous chemicals, including but not limited to acetonitrile, methanol, 0.1N hydrochloric acid solutions, etc. were not required to wear appropriate eye protection (e.g., chemical splash goggles or equivalent); on or about 7/10/14.
