29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system.  Location - 761 Broadway Street, Everett, MA; On or about 8/20/2014, an employee installing siding from a ladder jack scaffold that was greater than 10 feet above a lower level was not protected from falling to the ground below.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system.   Location - 761 Broadway Street, Everett, MA; On or about 8/20/2014, employees installing siding from a roof that was greater than 6 feet above a lower level were not protected from falling to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access.   Location - 761 Broadway Street, Everett, MA; On or about 8/20/2014, employees accessing a roof to install siding were using a ladder that did not extend three feet above the working surface in which the employees were accessing.
