29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material being cut: (a) Carpenters Shop; DeWalt radial arm saw, Model 3526-76, Serial Number 82040066, the lower portion of the blade was unguarded, leaving the blade exposed to a height of approximately 4 1/2 inches.

29 CFR 1904.29(b)(1): The log of occupational injuries and illnesses, (OSHA Form No. 300), was not completed in the detail provided in the form and the instructions contained therein: (a) U.S. Department of Defense, Department of the Army - 15000 Hwy 115, Little Falls, MN. On or about August 21, 2014, the employer did not correctly and completely enter workplace injuries or illnesses on the OSHA Form 300 for the calendar year 2014. Two versions of the log were provided. The cases contained on the logs were not classified by placing an X in column G, H, I, or J; and the number of days away from work or days of on job transfer or restriction were not entered where applicable.
