29 CFR 1926.451(e)(1): Cross braces were used as a means of access and egress:    On or about August 22, 2014, and at times prior thereto, employees accessed the second and third tiers by climbing the cross braces where the employer did not ensure that there was a ladders installed on the four tier tubular welded frame scaffolding, exposing the employees to a fall hazard of approximately 21 feet.
