29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:L On or about 21 August 2014 at the job site located at or near 2945 Parkwood Road and Red Wolf Drive, Jonesboro AR 72401 the employer did not ensure employees working at the edge of the roof were protected from falling greater than 10 feet to the unfinished ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: Employees used a nine foot A-Frame ladder to access the working surface of the roof. The ladder extended about a foot below the bottom of the rafters and at least two feet below the working level of the roof. Employees were exposed to falls greater than 10 feet to the concrete floor below.
