29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    Employees used a nine foot A-Frame ladder to access the working surface of the roof. The ladder extended about a foot below the bottom of the rafters and at least two feet below the working level of the roof.  Employees were exposed to falls greater than 10 feet to the concrete floor below.
