29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  a) Slawonir Radgowski - On or about August 14, 2014, two employees working from a pump jack scaffold more than 10 feet above a lower level was not protected from falling by use of a guardrail or personal fall arrest system.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  a) Slawonir Radgowski - On or about August 14, 2014, two employees were working from a pump jack scaffold more than 10 feet above a lower level without the vertical poles being secured at ground level to prevent them from movement.
