29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. On or about August 19, 2014, at the center of the jobsite, the employer allowed the employees to operate the Perforator Roller (model #M616NEX) with inadequate strain relief. The outer protective jacket was coming loose, exposing a 4"-5" section of the wires.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated. On or about August 19, 2014, at the center of the jobsite, the employer did not ensure that the cord attached to the equipment was visually inspected for defects prior to use.

29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made. On or about August 19, 2014, at the center of the jobsite, the employer allowed employees to operate a Perforator Roller with a damaged powered cord. The damaged powered cord were wrapped with electrical tape in two (2) 6"-8" sections.
