29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) Jobsite, Burgatory-Blue Spruce, Murrysville, PA:  On or about 08/19/14 an employee was positioned on the top of the parapet installing Z channel and was not protected from falling by guardrail systems, a safety net or personal fall arrest system.  The fall hazard was approximately 22 feet to the ground below.

The employee was working in an aerial lift without wearing a harness and lanyard tied off to the boom or basket.

29 CFR 1926.503(c)(3): Retraining was not provided when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill:   (a) Jobsite, Burgatory-Blue Spruce, Murrysville, PA:  On or about 08/19/14 the employer did not retrain employees on fall protection when their work performance indicated the employees had not retained the requisite knowledge or skill to prevent falls.  An employee was working on the top of a parapet, approximately 22 feet above the ground, and no fall protection system was in place indicating the employee failed to retain the skill necessary to prevent a fall.
