The employees involved in controlling hazardous energy for this servicing/maintenance project were not adequately trained to recognize, isolate and verify (isolation of) all hazardous energy sources. 29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished:     a) On or about 08/04/2014 in the Tedlar casting basement; where maintenance work to replace a valve and sight glass proceeded without verifying if there was any hazardous energy at the valve. NO ABATEMENT CERTIFICATION REQUIRED    The employer, The DuPont Company, was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.147(c)(4)(ii)(B)), which was contained in OSHA inspection number 315004101, citation number 1, item number 1 and was affirmed as a final order on 11/02/2011, with respect to a workplace located at 3115 River Road, Buffalo, NY 14207.
