29 CFR 1910.305(g)(1)(iii): Flexible cords and cables were used for purposes prohibited by subparagraphs (A) through (E) of this paragraph. On or about August 25, 2014, and times prior thereto, the employer used flexible cords attached to the ceiling and connected by a 4 way junction box as a substitute for fixed wiring for the use of welding machines, grinders, fans and small electrical tools.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about August 25, 2014, and times prior thereto, in the maintenance bays, the employees used an Max air-flow fan with the power cord missing the ground pin exposing employees to electrical shock hazards.
