29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms:    (a) East Side of Building - Employees worked from the third buck of the fabricated frame scaffolding.  The north end of this working level, 18 feet above the ground, had only a single rail rather than a complete guardrail system consisting of a top rail and an intermediate member, exposing the employees to the hazard of falling to a lower level, on or about August 26, 2014.    No abatement certification or documentation required.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) North Side of Building, Roof over First Floor Office Area - An employee worked from the roof over the first floor office area to lay block exterior finish.  The employee did not have any fall protection system, exposing the employee to the hazard of falling from the roof 12 feet to the ground below, on or about August 26, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
