The north end of this working level, 18 feet above the ground, had only a single rail rather than a complete guardrail system consisting of a top rail and an intermediate member, exposing the employees to the hazard of falling to a lower level, on or about August 26, 2014.    No abatement certification or documentation required.

The employee did not have any fall protection system, exposing the employee to the hazard of falling from the roof 12 feet to the ground below, on or about August 26, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
