29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables were not protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.  At west side of the building structure; employees operated a mortar mixer with a damage flexible electric cord, exposing employees to electrical hazards.

Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  Inside of the building structure, east side; employees used an energized flexible cord without strain relief to power a mortar mixer, exposing employees to electrical hazards.
