29 CFR 1910.1030(d)(4)(ii)(A): Contaminated work surfaces were not decontaminated with an appropriate disinfectant immediately or as soon as feasible when surfaces were overtly contaminated or after any spill of blood or other potentially infectious materials:    (a) On or around August 20, 2014, when a blood spill incident occurred in the fitting room area, store workers were provided with two partially stocked Bloodborne Pathogen clean-up kits, which did not contain appropriate disinfectant to decontaminate affected surfaces.

29 CFR 1910.1030(g)(2)(vii): The employer's training program did not contain the minimum elements required by 29 CFR 1910.1030(g)(2)(vii)(A) through (g)(2)(vii)(N):   (a) With respect to bloodborne pathogens,  adequate training was not provided.  Among the elements not covered by the training were the following:    - access was not provided to a copy of the standard 29 CFR 1910.1030                - explanation of the epidemiology and symptoms of bloodborne diseases  - information on the hepatitis B vaccine, including information about its efficacy, safety, method of administration, the benefits of being vaccinated, and that the vaccine would be offered free of charge  - an explanation of the procedure to follow if an exposure incident occurs, including the method of reporting the incident and the medical follow-up that would be made available  - Information on the post-exposure evaluation and follow-up that the employer is required to provide for the employee following an exposure incident.
