29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.         Building 2, North wall:  On or about August 8, 2014, employees were exposed to fall and struck by hazards on a 3 level tubular scaffold while installing masonry to a three story apartment complex and there was no fabricated plank used on the scaffold.  Each of the scaffold boards were of varying width and some were broken and double stacked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.         Building 2, North wall:  On or about August 8, 2014, employees were exposed to fall hazards on a 3 level tubular scaffold while installing masonry to a three story apartment complex and there was no ladder installed for employees to access the upper levels.

Building 2, North wall:  On or about August 8, 2014, employees were exposed to a fall and struck by hazard while laying decorative stone on a three story apartment building and the scaffold was erected by a masonry subcontractor who was not a trained competent person and there was no competent person on site.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:         Building 2, North wall:  On or about August 8, 2014, employee was exposed to a fall from approximately 11 feet high to the ground below while laying decorative stone on a three story apartment building without any fall protection, the employee was straddling on the masonry on the building and the scaffold.
