29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  Building 9:  On or about August 8, 2014,  employees were installing wooden decking on a 3 story apartment complex roof with a pitch of 6:12 and were exposed to a fall greater than 6 feet to the ground level below without any type of fall protection.
