OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees to struck-by and caught between hazards: a) Facility - On or prior to August 27, 2014, employees were exposed to struck-by and caught between hazards while work on and around a 950 Loader (Unit # 9086) that did not have a functioning back up alarm, had a seat in bad condition with no seat belt, and an air light control that was not functioning that would indicate if there is enough pressure to use the brakes.

The 'Murph' or baler has more than one places to lockout the equipment depending on what part of the machine needs to be serviced and/or receive maintenance.
