29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a)Patty's Framing LLC at 10264 Telluride Way, Commerce City, CO 80022:  On and before August 23rd, 2014, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  The condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a)Patty's Framing LLC at 10264 Telluride Way, Commerce City, CO 80022:  On and before August 23rd, 2014, the employer did not ensure that employees were protected from falling when framing while working on the lower sheathed roof with a pitch of about 3 in 12.  The condition exposed employees to a fall hazard of 13 feet.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    (a)Patty's Framing LLC at 10264 Telluride Way, Commerce City, CO 80022:  On and before August 23, 2014, the employer allowed the employees to use an extension cord that was not grounded in the use of an 120 volt Dewalt circular saw, model DWS535T, which was not double insulated.  The condition exposed employees to electrical shock hazards.
