29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace. Employees were exposed to a fall hazard when wearing a frayed fall restraint belt, which was required while operating the order picker, and the employer did not ensure that employees were not using defective or damaged personal protective equipment. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
