29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about August 27, 2014 on a commercial job site, an employee working from a JLG aerial lift was not wearing personal fall protection nor tied off to the basket exposing the employee to a fall hazard of approximately 12 feet to the ground below.
