29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): The employer does not ensure that all working levels on scaffolds are fully planked or decked. This violation was occurred on or about August 27, 2014, when employees were exposed to a fall hazard while performing stucco work on the second level of a scaffold without the platform fully planked or decked between the front uprights and the guardrail supports. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all working levels on all scaffolds are appropriately planked or decked.

29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: The employer does not ensure all scaffold platforms are at least 18 inches wide to prevent employee injury. a. This violation was observed on or about August 27, 2014, on the north facing side of the new home being constructed, when an employee was exposed to a fall hazard while working on a scaffold platform that was not 18 inches wide. b. This violation was observed on or about August 27, 2014, on the east facing side of the new home being constructed, when an employee was exposed to a fall hazard while working on a scaffold platform that was not 18 inches wide. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds are constructed an maintained in accordance with 29 CFR 1926.451 or 29 CFR 1926.452.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: The employer does not ensure that a guardrail system is installed on all working levels of scaffolds. This violation occurred on or about August 27, 2014, on the north facing side of a new home being constructed when employees were exposed to a fall hazard while on the second working level of a scaffold without a guardrail system in place along all open sides. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffolds used by employees have a guardrail system installed that protects employees from falling from all open sides and ends of the platform.
