29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a) An extension cord used by employee in the sign shop was missing a ground plug exposing employees to a shock hazard. Abatement verification is not required for this item. The cord was cut in pieces and removed from service. Quick Fix.
