29 CFR 1926.501(b)(13): Employees engaged in residential construction activities 6 feet (1.8m) or more above the lower level were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 29, 2014, employees were preforming residential roofing activities at heights greater than six (6) feet above a lower level without using any method of fall protection, exposing the employees to fall hazards.

29 CFR 1926.503(a)(1): The employer did not provide training for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.  On or about April 29, 2014, employees engaged in roofing activities at a height greater than six (6) feet had not been provided fall protection training, exposing employees to fall hazards.

On or about May 29, 2014, employees were using portable ladder(s), to access a steep roofline, which was not leveled properly. Employees were exposed to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper level landing surface the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, and/or the ladder was not secured at its top to a ridged support that would not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.  On or about April 29, 2014, for the following instances:  a)  one (1) portable ladder was propped against drywall of house and was not secured to an upper landing surface b)  one (1) portable ladder was being used which was not at least 3 feet (0.9 m) above the upper landing being accessed  These exposed employees to fall hazards.
