29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about August 27, 2014, employees assisting with electrical activities from the top of a trailer were exposed to an unprotected fall hazard of approximately 8'4" to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed. On or about August 27, 2014, an employee used a 6 foot fiberglass "A" frame ladder to work on a Conex Trailer. The ladder was folded shut and leaned against the conex trailer exposing the employee to a fall hazard.
