29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    a)  On August 20, 2014, in the first floor café, employees worked with corrosive chemicals to include but not limited to Phosphoric Acid without access to an emergency eyewash station.     b)  On August 20, 2014, in the second floor espresso bar, employees worked with corrosive chemicals to include but not limited to Phosphoric Acid without access to an emergency eyewash station.     c)  On August 20, 2014, in the third floor espresso bar, employees worked with corrosive chemicals to include but not limited to Phosphoric Acid without access to an emergency eyewash station.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:    a)  On August 20, 2014, in the maintenance shop area, employees were exposed to the health and chemical hazards of an unknown chemical in an unlabeled container.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:      a)  On August 20, 2014, in front of the Visual Setup Office, an orange extension cord, Model 62206, was plugged into a Relocatable Power Tap (RPT), Model SUPER7TEL15 which was then plugged into the duplex wall outlet.
