29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about August 28, 2014, during the installation of water line pipe the employer did not ensure employees working inside an approximately 6.5 to 7 foot deep trench were protected from cave-ins by protection system. R.S. CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.652(A)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 982145 , CITATION NUMBER 01, ITEM NUMBER 002, AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 19, 2014 WITH RESPECT TO A WORK PLACE LOCATED AT 25140 WOLF ROAD, BAY VILLAGE, OHIO.
