29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a)Five Star Framing at 7128 Shadow Ridge Drive, Fort Collins, CO 80525:  On and before August 23rd, 2014, the employer did not ensure that employees wore safety glasses when using or working in the vicinity of pneumatic nail guns.  The condition exposed employees to the hazard of projectiles to the eyes.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    (a)Five Star Framing at 7128 Shadow Ridge Drive, Fort Collins, CO 80525:  On and before August 23rd, 2014, the employer did not ensure that employees were protected from electrical shock hazards in the use of a Skilsaw model HM7700 serial number HH-881574 that was missing its ground plug, and which was not double insulated.  The employer also provided a yellow extension cord, used with this Skilsaw, that was missing its ground plug.  The condition exposed employees to 120 volt electrical shock hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or a personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a)  Five Star Framing at 7128 Shadow Ridge Drive, Fort Collins, CO 80525:  On and before August 23, 2014, the employer did not ensure that employees were protected from falling in the following instances:    1)  The stairway hole to the basement, measuring 89 inches long and 40 inches wide, with a 10 foot fall hazard, was not covered or guarded;    2)  The second floor landing, measuring 52 inches on the front edge edge, plus 40 inches over the stairway hole to the basement, was not guarded, with a 20 foot fall hazard;     3)  Six windows on the second floor included window sills 36 inches high, and these wall openings did not include an upper guardrail, with falls up to 10 feet; and    4)  Three basement window wells, each measuring 60 inches long and 36 inches wide, with fall hazards of up to 6 feet 5 inches, were not guarded.     The employees were not tied off, and were not using other means of fall protection.  Conditions exposed employees to fall hazards of up to 20 feet.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:    (a)Five Star Framing at 7128 Shadow Ridge Drive, Fort Collins, CO 80525:  On and before August 23rd, 2014, the employer allowed employees to use the top and top step of a 6 foot stepladder to access the second floor.  The condition exposed employees to fall hazards of up to 20 feet.
