29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about August 27, 2014 and times prior thereto, an employee of Gabriel Rosas was observed performing repairs on a rooftop of a single story residence located at or near 6225 Alan Drive in Jonesboro, Arkansas. The employee worked on the peak of the roof which was steep-sloped with a pitch of 12:12 and had unprotected sides and edges greater than 6 feet from ground level with no means of fall protection, exposing the employee to fall hazards.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. On or about August 27, 2014 and times prior thereto, an employee of Gabriel Rosas was observed working on a rooftop of a single story residence being repaired at or near 6225 Alan Drive in Jonesboro, Arkansas. The employee was observed nailing plywood to the peak of the steep sloped rooftop with a pitch of 12:12. The employee had not been trained in fall hazard and the employer had not certified compliance with 29 CFR 1926.503(a).
