29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:     a)  Small Press Area:   On or about September 16th, 2014 the employer exposed employee(s) to noise at levels of 145 % (equivalent to an 8 hour time weighted average TWA of 92.68 db) which is 1.45 times the Permissible Exposure Limit of 100 % (equivalent to an 8 hour TWA of 85 dBA) while operating the dismantle saw.  Results were calculated for a 271 minute sampling period. Dosimeter was removed before shift over due to overexposure documented.  The employer failed to conduct audiometric testing.    b)  Spare area press:   On or about September 16th, 2014 the employer exposed employee(s) to noise at levels of 89.9% (equivalent to an 8 hour time weighted average TWA of 93.1 dBA) which is 0.56 times the Action Level of 50 % (equivalent to an 8 hour TWA of 85 dBA) while operating Simpac MC2-300 press in the spare area.  Results were calculated for a 279 minute sampling period.  The employer failed to conduct audiometric testing.     c)  Large Press area and Robotic Press area overhead crane operator:   On or about September 16th, 2014 the employer exposed employee(s) to noise at levels of 80.20% (equivalent to an 8 hour time weighted average TWA of 88.41 dBA) which is  0.62 times the Action Level of 50 % (equivalent to an 8 hour TWA of 85 dBA) while operating overhead crane.  Results were calculated for a 275 minute sampling period.  The employer failed to provide audiometric testing.
