29 CFR 1910.95(g)(6):At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) 1000 ton press operator: On or about May 20, 2021 an employee was exposed to noise dose at levels of 176.4 percent (equivalent to an 8 hour time weighted average TWA of 94 dBA) which is 1.76 times the Permissible Exposure limit of 100 percent (equivalent to an 8 hour TWA of 90 dBA) while working at the 1000 ton press machine and was not provided an annual audiogram. b) 700 ton press operator: On or about May 20, 2021 an employee was exposed to noise dose at levels of 137.4 percent (equivalent to an 8 hour time weighted average TWA of 92.2 dBA) which is 1.37 times the action level of 100 percent (equivalent to an 8 hour TWA of 90 dBA) while working at the 700 ton press machine and was not provided an annual audiogram.
